This form is designed for use by physicians to complete the medical screening of a volunteer emergency responder (firefighter and EMT). It is best to provide the Guidance to Physician's referenced and the NFPA Standard 1582 to the physician fin order to gain a better understanding of the duties performed by the firefighter.
